First things first,if you care so much about her,make her feel safe and secure.
She started getting distance because you were a Hulk while intoxicated:It's been 3 years,cut those stuff, no matter what.
Secondly,you broke her trust by letting the cat out of the bag,especially when she was secretly keeping tabs with you.
I might be wrong,but she was longing for you to gather momentum and make things right,but you were too afraid to engage ,as you describe it.

The solution is tough to accomplish ,yet laughably simple.
You STILL have a chance.
Step up,and make her feel secure.

How you'll do that,you ask?
Easy,cut the stuff that make you lose control,namely drugs and alcohol.You see the results of their abuse.
Don't say that you are good now etc...show it.Let her see on her own what you are capable of doing/achieving.Be firm and honest,show that you still care and only want what's best.

Get it together,and get back in the game facing the facts you already know.There are no shortcuts here.
