Most Unethical Thing Youve Ever Done?

In hindsight what I'm about to describe isn't necessarily something I could have prevented, but I still feel crap about it.
Shortly speaking, I failed to notice that my good friend was in need of help as I myself was really depressed, and the person ended up taking their own life.
