Conversation-like fillable form

I am going to create this regardless, but I have a small job to do and I'm wondering if there's a script or something I can easily build off of. Basically the client wants a fillable form/questionnaire but wants it to look like a conversation. Maybe the layout would look like a text conversation, but overall it's just aquestion(other person's text bubble),then fillable text (your bubble). If there was something already made like this that you know of please let me know as it would be neat if there was something a little more interactive or stores information, etc. I was even wondering if there's an automated live chat support system I could manipulate/dumb down to do what I need it to? Hmm.I will end up creating a basic html script to do this if anything.
