Description
This is a simple app which allows you to create unlimited application forms for you board. You're able to configure the fields for each form. The application can also change a members primary or secondary group, once it was approved by the administrator.
Once the form was submitted, a topic will be created in the specific forum.
Application Topics can have a poll attached with Yes/No as voting options.

Can you add as many questions as you want? Or is there a limit to how many you can add. Also is there a way of setting a specific answer and if they get the answer wrong they must re do the questions?
