A). checks what is currently running in the background of your computer (ex. cmd's, applications, web browsers)
B). VAC may identify something as a cheating client and blacklist it. It will then flag any account that has it downloaded. It may not actually ban those accounts for a few weeks or even a few months, this feature is used to make people and coders think their cheat may have finally bypassed VAC and is FUD (fully undetected). The longer it goes the more the coder will advertise it the more people will use it and thus the more people who will get banned.
Even paid clients won't make it too long.

Most private cheats are undetected for a very long time. VAC just sucks :(

Referring to "A", VAC detects .dll files that have been injected into the game, but you won't get VAC'd or untrusted if it's not detected. You can make ANY cheat undetected by changing some of the signatures of the cheat and encoding it (doesn't matter if it's binary or not). Now to "B." I am in non-prime. There's a statistical fact that there's a 70% chance of having a cheater in your game. These were just the facts about cheaters that were banned, can you imagine the ones that weren't? "Closet" cheaters (cheaters who hide or attempt to hide it) are normally decent at the game but they cheat because, well, it's fun. These people dominate in prime and non-prime (mostly in higher ranks but whatever the case they're still there). It's really late and I am tired, good night.
