It's usually just lua scripts on servers with bad configuration. I used to have a few scripts on my hardrive but I wiped it because I upgraded to an SSD. Usually you can find some lua scripts online that give you wallhacks, aimbot, esp, etc. If a server has sv_allowcslua set to 1 you can use those scripts.
