Here are some page I completed in my free time today. From what I've seen no one designs these pages so I took a crack at it, tell me what you guys think.

What forms have you used or did you make them yourself?

Sorry yeah I made them myself after seeing this type of style on some forums login pages (including nulleds). Thought it would add something extra to an other wise plain textbox.

Believe it or not you where not the first/only one to do something like this, it's pretty common

It's pretty obvious. Same height, same padding and same margin on the spacer. And if I'm wrong, post the source here and we'll see. :spooky:

I will post the source if you'd like however I just followed this tutorial here as I didn't know how to get an icon inside. I did use the same icon and the idea did come from you.
