[REQUEST] Application Forms

Hi,
I'm looking for the Application Forms app.
I found one on this site, but link is currently down. What I need is forms with multiple option to select (mostly radio buttons or text area). I was trying to write this as HTML, but it doesn't work correctly, I mean it doesn't send a PM to selected member or e-mail.
