Nasyr, you didn't include the root file in the download, you only put the one which is in inc/plugins.
There's 2 selfban.php in this.

This one goes into your root MyBB installation & the one nasyr shared goes into inc/plugins
Name the root file selfban.php aswell.

now do www.yourforumurl.com/selfban.php

Also go to forum general settings -> scroll all the way down to selfban settings to set which usergroups can access it.

One bug is there, if i kept allow group to all than its not working.
