Instead of making 15 points in your forum it is easy to buy 1 from official website .? what you want by posting and forcing users to post 15 .?
supose i have nothng to post and i came here just to download a fucking plugin ? what should i post ..

Description
This is a simple app which allows you to create unlimited application forms for you board. You're able to configure the fields for each form. The application can also change a members primary or secondary group, once it was approved by the administrator.
Once the form was submitted, a topic will be created in the specific forum.
Application Topics can have a poll attached with Yes/No as voting options.
